Michaels Health Screening

To help you achieve and maintain good health, Michaels is providing you with health and wellness opportunities. Michaels Team Members enrolled in Kaiser Permanente benefit plan and Michaels Team Members and spouses/domestic partners enrolled with Aetna that do not complete their preventive exam  under the Aetna plan are eligible to participate in the LiveWell program. 

To participate in the LiveWell program, complete the Annual Physical Form before June 30, 2023 to avoid a surcharge on your 2023 medical premium beginning August 2023.

Team Members with medical coverage beginning March 1, 2023 or after will not need to complete the wellness requirement.
